Hi,
Be really specific about what it is you find difficult for example:
Even though I find C hard to play because my little finger is a little stiff and finds it hard to press down correctly and firmly on the fret, I completely accept myself
Which chords are the most difficult? What chord is the most difficult to play following another? Think about things like dexterity, being musical, being quick, having tempo etc etc.Get down to the nitty gritty of playing. Think about how you feel about finding it hard to play again, are you making judgements about yourself like "I should know this" "I can't believe I forgot how to do this" "I used to be able to do this chord change so much faster", tap on these first.
Have a look at the tutorial on the importance of being specific.
