Hi,
Since you can remember the very first time your swallowing issue started, why don't you take yourself back to this event and tap on all the feelings you have about it? This is a very useful way to make real progress on the issue. If it's very intense for you even thinking about thinking about it, you can tap on that feeling first, until you feel more confident to tackle the actual memory itself.
When you feel more confident about handling the memory itself (if this has even been an issue for you), think about what the experiences meant for you, and what beliefs you still hold about yourself as a result of that experience. Forming tapping phrases around these ideas will hopefully be more effective than tapping on the memory alone.
You can also tap at work whenever you get the chance; using the finger points is a subtle yet effective way to get EFT done when you're in situations like those. Try not to prevent yourself from swallowing or controlling your behaviour too much, but just use the tapping before, during and after the swallowing - basically whenever you get the opportunity. You don't really have to think about what to say.
